Once forms are filled out please email newaccounts@nethealthclaimservices.com or fax to 909-307-8510 attn: New Accounts. You will be notified by a new accounts representative what the next step is to be able to get you up and running.

To setup  ONLINE PATIENT PAYMENTS go to the Merchant Services setup link and fill out the online application and also, fill out:
